Request for Review Form

The legislative mandate of the Military Police Complaints Commission of Canada (MPCC) is found in Part IV of the National Defence Act, "Complaints About or by Military Police."

A complainant who is dissatisfied with the investigation or disposition of their complaint by the Canadian Forces Provost Marshal (CFPM) may refer the complaint in writing to the MPCC for review.

Please note: Use of this form is not required; you may also transmit your request for review to the MPCC by email to commission@mpcc-cppm.gc.ca.
